Service Agreement Request Form

(Formerly Independent Contracts)

Sites/Departments complete the "Service Agreement Request Form" and submit completed form to the the appropriate Division for approval to go forward for approval by the Board of Education.

Elementary Schools submit to Executive Director, Elementary.

Secondary Schools submit to Executive Director, Secondary.

Master Contracts

The following vendors have "Master Contracts" with the Newport-Mesa School District.

When using one of these vendors, a Service Agreement does not need to be completed.
