What information is needed to complete the pre-enrollment process?

The online Pre-enrollment may be completed at home on your computer or at the Welcome Center, located at:  2045 Meyer Pl, Room 112, Costa Mesa 92627.  You will need the following information to complete the online pre-enrollment or the printed forms:

  • General information about the student
  • Student's Birth Certificate
  • Parent's valid Identification card
  • An email address (if you don't have one, we'll create one for you)
  • Name, address and district of previous schools - Students entering grades K-6 are also asked for pre-school information
  • Report Card (Grades K-8)/ Transcript (Grades 9-12)
  • Parent/Guardian's home/work phone number and address
  • Name, address and phone number of emergency contacts
  • Student health & dietary issues, medications taken
  • Immunization dates:
    • Polio (OPV or IPV)
    • DTP/DtaP/Td
    • MMR (Measles, Mumps, Rubella
    • HIB Meningitis
    • Hepatitis B
    • Varicella (Chickenpox)
  • One out-of-state (within the USA). contact
  • Local physician name and phone number
  • If the student was not born in the USA:
    • Date student entered USA
    • Date student first enrolled in any USA school

Once you have completed the online pre-enrollment, please go to the Welcome Center at the BESST Center to complete your child's pre enrollment packet. For more information, please call 949-515-6740.
