School Health & Immunization Requirements
To enter school, parents must present a signed or stamped immunization medical record from a physician or clinic. Students are required to meet all state immunization requirements. Students who have not yet completed the immunization series are allowed to attend school only on condition that they are in progress and up-to-date in the sequence of required immunizations.

(TO ACCOMPANY ORAL HEALTH ASSESSMENT/WAIVER FORM)
Dear Parent or Guardian:
To make sure your child is ready for school, California law, Education Code Section 49452.8, requires that your child have an oral health assessment (dental check-up) by May 31 in either kindergarten or first grade, whichever is his or her first year in public school. Assessments that have happened within the 12 months before your child enters school also meet this requirement. The law specifies that the assessment must be done by a licensed dentist or other licensed or registered dental health professional.
Take the attached Oral Health Assessment/Waiver Request form to the dental office, as it will be needed for your child’s check-up. If you cannot take your child for this required assessment, please indicate the reason for this in Section 3 of the form. You can get more copies of the necessary form at your child’s school or online from the California Department of Education website. California law requires schools to maintain the privacy of students’ health information. Your child’s identity will not be associated with any report produced as a result of this requirement.

I. IMMUNIZATIONS FOR SCHOOL

PRESCHOOL PHYSICAL EXAM reports shall not be more than 1 year old upon day child starts school each year.
III. TUBERCULOSIS SCREENING:
Proof of screening for risk of tuberculosis one year prior to or within 30 days following enrollment.

II. SCHOOL PHYSICAL EXAM AND TUBERCULOSIS REQUIREMENTS FOR TK/KINDERGARTEN AND FIRST GRADE:
All enrolling kindergarten students (including transitional Kindergarten, TK) are required to present a certificate by a health examiner certifying that the student has completed a comprehensive health screening and screening for risk of tuberculosis within six months prior to entry into TK/kindergarten or within 90 days thereafter. A first grade student who has not previously enrolled as a kindergarten student will need to show proof of a health screening and screening for risk of tuberculosis within 18 months prior to entry or within 90 days thereafter. This comprehensive health screening for school entry may be free to those who qualify through the CHDP (Child Health and Disability Prevention) program. A waiver signed by the child’s parents or legal guardian, indicating that they do not want or are unable to obtain such health checkup for their child, will be accepted in place of the certificate.
